About The Vision Benefits Program

The American Samoa Medicaid State Agency Vision Coverage Program began in 2022. It was designed to expand optometry services outside of LBJ to provide comprehensive eye exams, contact lens fittings, and eyeglasses etc. for eligible beneficiaries.

From 2022-2024, the Medicaid Vision Program has provided coverage for over 1,700 eligible beneficiaries.

Beneficiary must enroll at the Medicaid office with all required documents to screen for eligibility.

Medicaid screens documents for eligibility and approval (1-3 business days)

If approved, Medicaid informs the beneficiary and the Vision Provider of the patients approval.

Once approved, the Vision Provider contacts beneficiary to schedule appointment and bill Medicaid after service is rendered.

ALL CHILDREN UNDER 21 are fully covered by the Medicaid Program once every two years

ELIGIBLE ADULTS are partially covered by the Medicaid Program with a copayment option of total cost of service exceeds Medicaid coverage of $450

Birth Certificate or US Passport

Two Valid AS Identification: Driver License, AS Voter's ID, AS Vitals ID, utility bill, telephone bill, Proof of Employment, Bank Statement or any other verifiable document.
*Any other documents required from Medicaid.
